Medicare Prescription Drug Plan Letters

Creditable Coverage Letter:
This letter is intended to communicate that the current prescription Rx coverage being offered is creditable. That means it is better than the new Medicare Part D coverage. Where it indicates 'Insert Name of Entity,' that is the name of your health carrier. Where it indicates 'Name of Plan,' that is the name of your health plan.
